register yourself to be a user of the Amber/Booth Catalog by filling out
and submitting this form. Only registered users are allowed to access the
catalog. This allows us to contact you when important changes are made to
our literature. (The information you submit when you register will ONLY
be used by Amber/Booth for this purpose.) Once you have completed
the registration, you may use the username and password you are given to
access the Amber/Booth Catalog.
